Homeschool Evaluations

Florida law requires homeschool families to submit an annual evaluation showing that their child is making educational progress. One of the most common options is a portfolio review by a certified teacher. This checklist helps parents prepare a clear, organized portfolio that meets requirements and makes the evaluation process smooth.


Student Information

  • Child’s name, age, and grade level
  • Date range of portfolio (beginning–end of homeschool year)
  • Parent/guardian name and contact information

Work Samples

Include 3–5 samples per subject showing growth over time:

  • Language Arts: Stories, journal entries, reports
  • Math: Worksheets, problem sets, photos of hands‑on activities
  • Science: Lab notes, drawings, project summaries
  • Social Studies: Maps, timelines, essays, creative projects
  • Electives/Enrichment: Art, music, Bible study, foreign language, extracurricular activities

Reading Log 

  • List of books read (title + author)
  • Optional: child’s notes or short summaries

Parent Records

  • Lesson plans or weekly outlines (informal is fine)
  • Attendance log (days or hours of instruction)
  • Notes on progress, challenges, and achievements

Evaluator Notes

  • Space for certified teacher’s comments during review
  • Signature and date of evaluator

Optional Extras

  • Photos of projects, field trips, or activities
  • Certificates of completion (classes, sports, clubs)
  • Standardized test results (if applicable)

Helpful Tip: Organize the portfolio in a binder or digital folder with sections labeled by subject. Clear organization makes the review faster and easier for everyone.
